New Client Questionnaire

We ask that you please reschedule or cancel at least 1 day before the beginning of your appointment or you may be charged a cancellation fee. In the event that you are unable to make it to your appointment and have not rescheduled/cancelled within this 24 hour timeframe, the card used to book will be charged a $25 No Show fee. Please note that the amount charged will not be able to be used toward a future service. 

All forms related to services must be completed before scheduled appointments. Failure to do so could result in appointment cancellation.

In the event that you are running late to your appointment, please give me a call/text stating such and your ETA. A grace period of 10 minutes past your scheduled appointment is given. Arrival 11-14 minutes late will incur a $15 late Fee. After 15 minutes your appointment will auto-cancel and the card used to book will be charged the a $25 No Show fee. Please note that the amount charged cannot be used toward rescheduling of the service.
